（１）  参加したプログラムの内容及び主旨

　私は米国のVeterans Health
Administration Greater Los Angeles
Healthcare Systemに約2か月留学し、呼吸
器科に3週間、感染症科に2週間、放射線科
に2週間在籍し研修を行いました。呼吸器科
ではカンファレンス、病棟回診、外来、気管支鏡検査などに参加しました。
感染症科では同じくカンファレンス、病棟回診、外来に加え、他病院でのカンファレンスにも参加しました。放射線科では放射線科医のスタッフの先生に適宜読影の指導をして頂きました。

（２）  留学で学んだことを国立病院機構が提供する医療の質改善のためにいかに活
        かすか

　留学して感じたことは米国の医師は概してベーシックな知識は非常に豊富であるということです。ひとつには教育的なカンファレンスが多く知識を身につける場が多いからではないかと感じました。例えばVAでは若手医師を対象としたnoon conferenceと呼ばれるレクチャーが毎日あり、昼食を食べながら勉強をしていました。日本の病院では、各病院で異なるとは思いますが、カンファレンスを頻回にできるほど時間の余裕が無いことが多い様に思います。米国に比べ多くの患者を受け持ち、日々の仕事に追われ、学術的な側面がおろそかになりがちだと感じました。その分臨床経験を積めるメリットもあるとは思いますが、日本でも臨床の中で系統的なレクチャーがもっと組み込めればと感じました。また完全に当直医制で時間外に呼び出されることは無くon‐offをしっかりつけられるのも良い点だと感じました。この点に関しては、日本は主治医と患者の関係を重視する風土があるため、馴染まないかもしれません。しかし臨床においては日本の方が優れている点も多いように思いました。例えば上記の当直医制の例でいえば、夜に急変した場合でも日本では（少なくとも私の病院では）主治医が飛んできて治療にあたる事が多いと思いますし、亡くなった場合でも主治医が見送る場合が多いでしょう。治療の質はケースバイケースですが、少なくとも患者さんや患者家族にとってはこちらの方が満足度が高いように思います。こういった主治医患者関係という点では米国は非常にdryであり、主治医の負担は大きいですが日本の良い点ではないかと感じました。また気管支鏡の手技やデバイスは日本の方が良いように感じました。日本の医療はすべての患者に平等に高い水準の医療を提供しているという点で、世界に誇れるべきものだと思います。しかし日々の業務が多く、十分な学術活動を行う余裕が無い医師が多い様に思います。医師の仕事をある程度他職種に分担できれば、学術活動がさらに活発になり、日本の医療はもっと発展するのではないかと思いました。またVA hositalにはアジア、中東など様々な国の医師が在籍していました。皆英語で自由にコミュニケーションをとり、活発に議論しているのをみて、英語の重要性を感じました。英語を使えば世界すべての人とコミュニケーションをとることができる、そう思うと今後論文を書くにあたって、英語を使用し世界の医療者に発信していこうと思いました。

（３）  留学生活に関して、以後本制度にて留学する者が参考になる情報

　VA hospitalの近くに日本人街がありそこに行けば日本の食品を販売しているスーパーやラーメン屋、すし屋、カレー屋などがあります。ここ以外にもラーメン屋とすし屋に関してはLA内に多数存在し、店主が日本人の場合はたいていおいしいです。コリアンタウンの焼肉なども美味で、選べば食に困るということはない様に思います。

　アメリカは車社会なので、レンタカーがおすすめです、というより車がないとなかなか行きたいところに行けません。ただし右側通行含め若干交通ルールが違うので、事故をしないように気をつけてください。私は対向車のいない十字路を曲がるとき、つい左車線に入ってしまいそうになったことが数回あります、注意してください。

　現地には外国人向けの英語教室がいくつかあり、その一つに私は通っていました。費用も教科書代程度でした。英語の勉強という側面もあるのですが、それこそさまざまな国のさ まざまな年代の人たちとコミュニケーション がとれるので、その意味でよい経験になりました。

英文報告書

（１）  参加したプログラムの評価

 This training program is excellent because it gives us a great chance to learn an American medicine and culture. I think we should continue this program. VA hospital is a very good hospital to learn, but it is not the top in the US. So if you make a chance for us to see a most high-level one, I guess this program would be more exiting.

（２）  感想

In last summer, I stayed in the US for about two months and joined this training program. I learned American medicine at Veterans Health Administration Greater Los Angeles Healthcare System.

This hospital has a lot of department. So I think everyone who joins this training program can choose some departments they want. I chose the following departments, Pulmonary medicine, Infectious disease and radiology.

First, I visited pulmonary medicine. We discussed our patient in a resident room every day. I thought the practice is almost same. The way of diagnosis, treatment, drug et cetera, anything is almost same. But of course, there are differences. For example, they take a pulmonary function test more frequently than Japanese pulmonary physician instead of taking X-ray or CT scan. They usually treat Coccidioidomycosis which I’ve never seen because the disease is extremely rare in Japan. In addition, the medical system is quite different. I was surprised that they didn’t have any their own inpatients. Inpatient means the patient who stays in hospital. The patients belong with general medicine. So they only need to advise about consultations. In my hospital, if my patient gets worse even midnight or holiday, I have to go to hospital as soon as possible and treat him by myself. So I thought it was good system for pulmonary physician. In the clinic coincidentally, pulmonary fellows examine outpatients first, and next they consult attending doctors. Attending doctors advise how to treat or how to follow up. I think this system is very good for young doctors in education.

After 3 weeks stay in pulmonary medicine, I visited ID. ID means infectious disease. I saw endocarditis, diabetic foot, osteomyelitis and so on. Most diseases were related to DM. And I realized some antibiotics were a little different, for example, doxycycline, Tigecycline, eltapenem et cetera. This was the first time I learned at ID department and this fellow was very friendly and funny guy so I enjoyed this department very much.

The last 2 weeks, I visited Radiology, especially chest radiology. In this hospital, radiology was divided into 5 sections. For example, chest, breast, neurology, and so on.

Dr. Barack was an attending doctor in the department. He was an excellent chest radiologist. He taught me how to read chest X-ray. For example, how to read atelectasis and congenital heart disease like ASD or PDA. In Japan, we depend on CT scan too much. I thought we have to study chest X-ray more. I felt the importance of Chest X-ray. In addition he was very interesting parson. He was a big fun of sushi. He said he eat sushi more than once a week. It is amazing. But in fact, Los Angeles has a lot of nice sushi restaurants. One day he took me to his most favorite sushi restaurant named Kiriko. I was very surprised the sushi they served. The sushi was as delicious as that of a high-class sushi restaurant in Japan. I keep in touch with him even now.

From now, I introduce my American life. The apartment I stayed was more gorgeous than I had expected. Our coordinator rented it but this apartment has two pools and fitness! Of course, I used this fitness almost every day to do weight training. On weekend, I enjoyed sightseeing. I went to the Grand Canyon. I was impressed by the beautiful landscape. I sew The Golden Gate Bridge in San Francisco, too. This city was very beautiful. I also learned American culture through this trip. I guessed I would never have as much free time as this time. So I went to as many places as possible.

　Dr. Kaunitz, Dr.Akiba and many stuff helped us so we enjoyed US very much. I recommend for all young doctors
　working at NHO to join this training program. I’m sure that they will get a great experience like me.
