次のとおり一般競争入札に付します。

① 落札決定は、一般競争入札（最低価格落札方式）をもって行う。
② 入札金額については、仕様書に定める業 務の履行に要する一切の費用を含
めた額とすること。
③ 落札決定に当たっては、入札書に記載された金額に当該金額の１０パーセント
に相当する額を加算した金額（当該金額に１円未満の端数があるときは、その端
数金額を切り捨てた金額）をもって落札価格とするので、入札者は、消費税及び
地方消費税に係る課税事業者であるか、免税事業者であるかを問わず、見積もっ
た契約金額の110分の100に相当する金額を入札書に記載すること。
２ 競争参加資格
（１）契約細則第５条の規定に該当しない者で あること。なお、未成年者、被保
佐人又は被補助人であっても、契約締結のために必要な同意を得ているもの
は、同条中、特別の理由がある場合に該当する。
（２）契約細則第６条の規定に該当しない者で あること。
（３）厚生労働省競争参加資格（全省庁統一格） において「役務等の提供」の
Ａ、Ｂ、Ｃの等級に格付され、競争参加資格を有する者であること。
なお、当該競争参加資格については、令和８年3月31日付け号外政府調達
第58号の官報の競争参加者の資格に関する公示に掲げる申請受付窓口にお
いて随時受け付けている。
(４) 契約細則第４条の規定に基づき、経理責任者が定める資格を有する者で
あること。
(５) その他仕様書で定める受託要件に該当する者であること。

入札書に本公告 に示した物品を納入できることを証明する書 類を添付して
入札書の受領期限までに提出し なければならない。入札者は、開札日の前日
までの間において、経理責任者から当該書類 に関し説明を求められた場合
は、それに応じ なければならない。
(4) 入札の無効
本公告に示した競争参加資格のない者の提出した入札書、入札者に求められる
義務を履行しなかった者の提出した入札書は無効とする。

契約細則第21条の規定に基づいて作成された予定価格の制限の範囲内で最低
価格をもって有効な入札を行った入札者を落札者とする。
